There's another entire circle of stigmas inside the outer ones. This is going to sound really, really stupid but I assumed this one would abort... so I didnt even pollinate it as well as I should have. Ugh I can see what a fool I am now.

It does look mostly pollinated but I may have missed some of that inner stigma circle.

First and second rules of tomato growing:

1) Pollinate everything, assume nothing.

2) When in doubt, shake more pollen out!
